Life Maps

Life Maps* are developmentally appropriate transition questionnaires that allow families and young people to identify their own individual needs. They cover a range of topics from health promotion and health problem management to independence and work issues. The questionnaires encourage family-centered care that focuses on the individual needs of the young person.

Teaching Sheets are single pages on a variety of topics. Feel free to print them and share with friends, people at school and others.
