Family Financial Participation

A financial participation plan, based on a sliding fee schedule , will be established with each family.

Guidelines:

  • All families are financially eligible for CRS .
  • CRS clinic visits and care coordination services do not require financial participation on the part of families.
  • The family participation co-payment* will apply only to purchased services (hospitalization, X-rays, dental, hearing aids, orthotics, etc.) a family received during the previous calendar year.
  • The amount of financial participation is determined at the time of enrollment, annually, or as the family financial situation changes. A financial participation co-payment* is determined based upon family size and taxable annual family income.
  • Medicaid enrollees, ALL-Kids enrollees, Blue Cross Blue Shield Caring Program enrollees, and families with taxable annual incomes under 200 percent of the Federal Poverty Guidelines will not be charged any fees or co-payments.
  • No financial participation will be required for children with special health care needs who have been adopted through the State Department of Human Resources or who are in temporary custody/foster care.
  • Special guidelines may apply when a child resides in a related home without custody.
  • Families will be charged one family participation co-payment* amount per year regardless of the number of family members served by CRS .
  • A letter will be sent to the family by the district office specifying the annual financial plan.
  • Annual family participation co-payment amounts will be billed and collected by the  CRS State Office based upon the financial plan on record when the bill is generated.

*At this time, due to funding shortfalls, CRS has suspended payment of purchased services for families with taxable annual incomes above 200 percent of the Federal Poverty Guidelines except for clients who participate in the State Hemophilia Program.
